• EFT is still considered to be in the experimental stages

• EFT is not intended to replace advice or care from your healthcare provider

• EFT is not intended to diagnose or treat any particular individual

• My services should not be construed as medical advice or diagnosis

• The information and coaching I provide is educational and inspirational in nature and should not be viewed as a substitute for any treatment prescribed or recommended by your healthcare professional

• By participating in EFT, you agree to take full responsibility for your own physical and emotional well-being and agree to hold me (Lucy Superfox Majury nee Walton) harmless in the provision of EFT services to you.

I confirm that by electronically signing this form that:

1. I have read and understood this waiver

2. I choose voluntarily to participate in the EFT process

3. I agree to now and forever release (Lucy Superfox Majury nee Walton) from any and all claims that may, or which could arise, in connection with the services provided, or to be provided by (Lucy Superfox Majury nee Walton.) These claims include but are not limited to, claims for, or aggravation of any personal injury, accident, and or for any physical, mental, or emotional condition that may result from work done on and with.

This document, once signed by me, may be submitted electronically, or via facsimile, and any copy hereof may be accepted and treated as if it were the original hereof.
